All very bizarre. If your horse has been out of work for 2 years, and a bit older, it's not going to be able to get back in to work so easily. Muscles will have gone, fitness will have gone, things will have stiffened up. You need to start off slowly, even by doing in hand walking to build her...
Mine does this too. Even more so if they're over a year old. o_O. Try and make sure all your background apps are shut, and keep it in your pocket close to heat. I have a charge pack i keep and i can also charge in the car. Charge pack is handy if youre out for a long ride as its fairly slimline.
As above, get as much trimmed off the frog as possible, oxygen kills the nasty stuff too. Then slather it with what ever your using, or pack it with cotton wool slathered in it or whatever.

In terms of diet he...
How does he keep his weight? How much work is he in? I would personally ditch the pasture mix and the linseed. If he's a stereotypical cob Weight-wise then come the spring the sugar beet would go too.
I'll just give a bit of history into my gelding. He is shod in front only. He copes fantastically without hind shoes, but if you take the front shoes off he is incredibly sensitive. He has had the onset of laminitis in the past, and thankfully caught very early. This was May 2013 and June 2015...
