Sarcoids on a sheath are treatable - one of mine had them lasered.

Just a thought which I hope will not be the case but something to ask the vet about - an old mare I once owned had a mild stroke ( confirmed by vet David Langrish) and was very much like this - odd behaviour of this type during the actual event, and then exhausted afterwards.

My Belgian draft cross (not the horse in the pic) had GA in 2022 for colic surgery. They did warn me beforehand about the general issues around recovery immediately post-op, but didn’t make any particular comments re drafts. He was fine.
