how far did you miss it by?
it is worth speaking to your vet and saying you dont need them up to date for competition, so is it really neccessary to start all over again?
my vets happy to vacc every 18 months if you dont compete
how underweight is she?
wether to excersise on not depends on the answer to the above question, i am another one who does not give hard/bucket feed for eight gain. just hay and grass or haylage if thats whats avaliable

a needle is a foreign body and dirt can travel in with it so it does not have to be the vets fault (although of course sometimes it is), you may just have been unlucky
is your horse clipped, the horses at work fully clipped are wearing less rugs than that, it has been a lot warmer this last week so we have reduced rugs accordingly-are you sure she is not too hot. i would be very concerned if that many horses at the same time on the same yard were dropping...

we had a pony that was registered and papered and in the year she was 15, 2 dentists and a vet looked at her teeth. they declared her any where between 7-9 but no older. so all 3 were wrong and by a long way
