I don't think this is cut back enough but is a bridle that I made for my mare you can see it is slightly cut away around the ears. You could get a saddler to do something similar

Seems a lot for an older horse. There shouldn't be an awful lot of root left at his age as opposed to a younger horse that would have a lot more tooth in the skull/head. More likely to be a sedate and wiggle job than a ga. It may be an idea to get head x-rays to find out the situation
Seems ok if they fit right but can't bear it when a thick stable rug is stuck out the back of a turnout soaking up water and pulling on the horses withers.
I would maybe be worried how the horse would deal with treatment for the sarcoid if she needed it (it is often painful and cost could soon add up with IV sedation if that would work!), if she may have to be sedated for shoeing. May be totally unconnected though.
