    its caused by botflys. If you have any Fuciderm it will get rid of it. My horse had two patches on his lips and my vet knew what it was straight away.

    just a quick afterthought, maybe swap your hifi to dengie hi fi molasses free. also i would cut out the fibre pellets , just in case its too rich for his gut to cope with.     my Tb is on Naf's Pink powder, unmollassed alfalfa and unmollassed beet. If they have too much magnesium in the diet their droppings become loose. The body expels the excess this way. Check your calmer as these are mainly magnesium based and so it could be that he is getting over the optimum level.
