Advice on how to fill in an insurance vet fee claim please.

Hello,

I need to complete a vets claim form for my mare who had an incident out hacking the other day.

In the part:

Please give circumstances leading up to the illness/injury and the names and addresses of any other persons involved other than the person in charge of the insured animal at the time.

Do I put the whole essay long story of what actually happened, giving the full run of events, or do I keep it really brief to go with the 1 & 1/3 lines they leave for you?

I've never had to fill one of these in before.

Any/all advice welcome.

Probably depends on what happened, so if an accident where someone else may be at fault or just an injury as in tripped and fell over. I have generally put fairly short but concise and then put in to contact me for further info if needed.
 
I once put 'see attached document' in the relevant box and gave a full explanation on a separate sheet of paper.
 
I would just give a brief description of what happended. Hard to give example without knowing the circumstances but along the lines of "horse stumbled out hacking and then showed signs of lameness" or "horse spooked and did xyz". If someone elses horse caused the injury by kick etc or if a driver was at fault then list their details and involvement. You will need to provide a vet report to back up the claim so that will give more information, they just need a basic idea of under what circumstances the injury happened and any liability issues. They basically want to make sure you weren't riding sitting backwards with a mobile in one hand and burger in the other!
