BHS Members Questionnaire for Degree Student

As part of my second year degree I am required to collected survey responses from BHS Members and to write up my results.

I would appreciate it if the BHS Members on here would be able to complete my questionnaire; theres only 8 questions and it won't take very long.

Done, it's nice to have one of these questionnaires that I can actually complete, without thinking 'these questions are designed to give a specific result and don't apply to my situation'.
