BSJA people please help me again

Right I am going to do my membership in the new year. I am entitled to the Pony Associate Membership pack for £99. How do I go about purchasing this? I know I have to send off the forms for myself, the horse and my mother but it doesn't give you the option of the membership packs on the forms. It just asks you which membership you want... so how do I fill in the forms to get the package instead of filling it all in seperatley and not getting the discount that I should get

You fill in a form for each of you, enclose the appropriate identifying document and a cheque together with a covering letter stating you want the Pony Associate Membership package.
