I sent the passport for my CB back to the Society for changing into my name a couple of weeks ago [my first pure bred - am so :-D]. Is that enough or should I be e-mailing for a form as well?

You should have received a census form when your passport was returned in case you have more at home! Congratulations on your purchase.
