Having done a very quick google it does look like the TRH Stim is done alongside the insulin test too....

Mine had a test around 15 years ask where she was starved, but the vet came at 9am so not too bad. I vaguely remember her coming back after a couple of hours to do another one.

To anyone interested, an update!
So the bloods have all come back clear. Her ACTH test is practically the same as before at 27, so vet is happy to rule out Cushings. Her general blood screen also is clear and shows no raised white blood cell counts, no inflammation, nothing. Vet did stress that if there was a severe organ (like liver or kidney) disease going on then it probably wouldn't show on the screening.
His gut instinct is that it is management related. For various reasons I'm moving yards soon, so we've decided to give it a month in the new place on new forage and turnout and see what happens.
