My boy had an argument with the electric fencing in the field 12days ago. We think he got his hind leg caught over a strand and then pulled until it came free. He came in with a swollen tendon sheath although wasn't lame. I left him turned out and the swelling went down so I tried riding him again and it swelled up again so left him out a few more days then he had a gallop around and the swelling came back up again. At this point I put him on box rest and today I had the leg scanned.

He was sound on exam but has a tiny palpable bump on the lateral side of his DDFT about halfway down his right hind leg. The scan shows very mild disruption of the fibres here. Fortunately as he is sound and the injury looks mild the long term prognosis should be good. The vets have recommended turnout in a tiny paddock where he cant get up a canter, 1 month of 10min walking under saddle, 1 months of 20mins and 1 month of 30mins then rescan.

I have spoken to my physio and she has recommended daily treatment with shortwave ultrasound as the injury is superficial so should penetrate ok. I was also looking into spa treatment for him. Unfortunately this leg isn't covered under insurance so I'm reluctant to try anything unless I can find strong evidence to support it works. He is on a broad spectrum joint supplement already. Not sure what any of the tendon supplements have in them that might benefit him extra? Thought about cartrophen injections but very sketchy evidence these work and even at cost price would cost around £200. Anyone any experience of a similar injury and any alternative therapies that do work?

The opinion of the orthopaedics professor (who I won't name...) I recently listened to at a CPD day was that only two treatments had real evidence of efficacy for tendon injuries - time and rest. I have worked in a practice that used a lot of adequan and can't say I ever noticed a difference in treated vs. untreated horses. Same story with PRP in my hands. Stem cell therapy has shown some promise but is it worth ~£2000 of your money for a mild injury?
