Dermological Bacteria Infection

ust wondered if any of you had come across this before?

My friends horse is 28 and has been suffereing with this condition since the end of winter. It starts of as a small scab but he itches so much that he itches his fur off and rubs it until it bleeds. Its awful to watch as it really is a manic itch. She has turned him out 24/7 since the condition arose and it stops him from itching too much but he has still managed to itch himself raw under his jaw. The vet has been called in 3 times and they have ruled out mites and blood tests have returned clear. They've tried 2 courses of antibiotics and is now trying steroids. The vet said that due to the mild winter bacteria is rife at the moment. He just seems so miserable and seems to have really aged over the past few months.

My friend is at the end of her tether and doesn't know what else to try. Her fear is that it won't have cleared up by the winter when he has to be stabled. Do any of you have any advice??

Has he taken cultures from the wound?
If he can identify the predominant type of bacteria present in the wound he may be able to target it with specific antibiotics. Where exactly is the wound?
What does he scratch it on?

Skin lessions are obviously a secondary infection that even if itis treated will not solve the problem in the long run.
It sounds like an allergy to me....do the steropids had any effect? Any allergy signs before?
