Downloadable Medical Card Insert?

Does anyone know of anywhere that I can download a copy of the insert for a medical armband? Need one for the weekend and it's not a BE event so pretty sure the secretary won't have them available on the day.

you can't get downloadable BE medical cards but USA eventing have one on their site
