Extreme reactions especially when loading-eyesight related?

I have a 17 hh middle weight that has traveled no problem in my Bateson trailer (max 17hh) for 4 years. Last year out of the blue he reacted badly once loaded (just before putting back bar on) by flying out the back of the trailer at speed hitting his head and cutting his poll. It was totally out of character as he is Mr Chilled. He also started doing the same when coming out of his stable, not all the time but if someone walked past whilst he was coming out. I felt that there may be a problem with his eye sight so he had a full assessment from an eye specialist. There were no issues found although they can't diagnose reduced sight like we go to the opticians for.. I have since purchased a 511 which gives him plenty of room and we are still having the same problems. Sometimes he will go in sometimes not but all of a sudden he will react and fly out backwards when loading or partially loaded. It's actually quite dangerous. A lorry isn't an option so it's looking like a life of happy hacking is on the cards but thought I would ask you guys if any of you had experienced anything similar. Feel so dejected and don't want to put my poor boy through this stress. Ironically he did load in a 7.5T lorry for his eye assessment that was hired. I had considered some of the 'loading' professionals out there but feel little point if it's not behavioural but actually down to his eyesight. Thanks.

As he is doing it in the stable too i would be considering a full work up. He is maybe getting tweaked somewhere just at the point he is moving and then the pain causes him to react. Try and get a few videos of him going onto the box trying to get both the normal ok loading and the bad one, to see if there is anything that sticks out in how he moves etc.

Could he possibly have had a fright, on the original occasion? Something happening behind him, or a loud or strange noise, just as he was on the trailer? Shooting off backwards seems like a fear response to me, and the trouble is once he's done it it can become a habit, like a self fulfilling prophecy.
