I usually try and do these surveys but this is one of the worst I have looked at so will not be completing it as it is far too basic, I cannot see any way that you can learn more about factors causing or contributing to sweet itch from this, I also have no idea what substrate means in the context you have used it, I would suggest you rewrite it with more thought.
Environment need more definition than rural or urban, it is already well known that low lying fields near a water source will be an issue, diet, preventative measures internal or external, other than rugs, are not mentioned, all in all a poorly thought out effort compared with most.

I filled in the survey, yes agree with the poster above, can't see what you can learn from it.

And what FFS is a "Substrate"???? I've never heard of that, and I dare say neither will anyone else who's filling out the survey.
