How do you fit a running martingale ? Should know but can't think, is it

1] buckle to left side of neck
2] when attached to girth should be able to get hand width inbetween chest and strap to girth
3] hand width between neck and neck strap all way round
4] rings, not attached to reins should reach to
a) withers
or
b) top of neck ?

Please help me. It is being used to stop reins going over head when he head shakes.

Just a thought - why not use an irish martingale?

Fitting sounds perfectly fine, just make sure girth loop isn't twisted otherwise it'll rub
 
Everything as you say. But I though a hands breadth from end of ring (hold this on end of your thumb) and the little finger of your hand on the horses wither. Hand spread out wide.
