Food in sinuses, any positive stories?

Hi all. I look after an old horse (he predates required passports but he is around 33). Had the vet out in the week as he wasn’t eating and was low. He had a high temperature and he prescribed antibiotics and pain relief and he is pretty much back to normal. I’ve put him out on good grass to try and get some weight on him and he came in yesterday with bright green slime coming out of a nostril. It looks like grass. This has led me to believe that he has a hole allowing food to get into his sinuses so although the drugs are working now it is likely to recur. I’m going to discuss with the owner but just wondering if anybody has had this kind of thing treated successfully? Just looking to make the right decision. Thanks in advance.

I had this happen after tooth removal. There was a communicating hole which allowed food in.

He has a catheter fitted so his sinus could be flushed. Once the gums healed over it wasn't an issue anymore.

Has he had his teeth looked at recently? Have some fallen out leaving a hole?

Good to know, thank you. Yes, he had his teeth checked a few months ago and she was able to remove one and a half teeth just with her hand. But when the vet came out last week he looked in the mouth and couldn’t see anything obvious but we didn’t sedate and I hadn’t seen the “food” in his nose so we didn’t know the cause of the infection at that point.
