Funny shaped shetland-feeding or work?

Ok, this is going to be hard to explain without pics, so bear with me lol

Sister in law has a shetland. He's 5 years old and is a funny shape. She asked me for advice and I said I'd ask you lot!

He doesn't seem to be 'developed' over his back. He's well covered without looking overweight body wise, but has a big neck with a slight crest, it's just his back looks 'poor' if that makes sense?

This led me to believe it's lack of muscle?

He's on hay and kept in a yard with access to his stable the majority of the day, goes out for a couple of hours per day on ok(ish) grass. He has a hanful of plain chaff with a natural salt rock dumped in the bucket just so that he's had a bucket when the others do, the salt slows him down.

Would you say it was lack of feed or lack of muscle causing this?
 
Definatley lack of muscle, shetlands were bred to live on barely nothing at all, so his diets seems fine to me, about the same as mine. They are funny shaped anyway! usually big bellies
