what i do is, ask her to stop, if she doesn't stand square, i nugde her with the leg on the side of the one i want her to move if that makes sens, ie. if she leaves her front right slightly behind, i nudge her with my right leg on the girth, if its the hind i move my leg back a bit. if that doesn't work i keep nudging her to take a step forwards untill she gets it right, then a big pat and loads of fuss, also try to make her/him stand square every time you stop, not just when schooling, my mare has now learnt it so well that she stand square whether i ask her to or not. but this is just what works with my mare and im not sure if its the proper way to do it so i'll let the people that know more about dressage tell you the proper way :)

My horse has always been trained to stand square, halt square etc, but if he is ridden, when he is asked to halt, his head needs to have freedom, otherwise he will be unbalanced. and stop with legs unlevel.
A lot of riders just pull on the reins, without then relaxing the contact, it is this relaxing which allows him to use his head to balance himself.
