HGV D2 form?- help anyone.

Ive been for my medical today, and my D2 form was part filled in by the training company. However im unsure whether i need to prove identity? (Fill out pasport bit)
And im guessing i need to send a photo too?

Sorry being dense but they have ripped the other half off, thats got the instructions on it!

If you have a digital passport, I seem to think they will cross refer ID on that - mine is in the post, we'll have to compare notes and results! Am doing theory bedtime reading instead of Jilly Cooper!

I believe that unless you want to change any details on your photocard - ie the picture - no you don't have to send off a photo - but you do need to include photocard and green paper bit too.
