Hock spavins

Any advice would be great full, my cob has just turned 19 he has arthritis now it seems to be in his hocks pretty sure it's bone spavins vet gave me some Bute but didn't seem to do much for him, he's walking like a robot and looks pretty uncomfortable when he walks, there's now muscle wastage looking at his back end he's not walking as he should be there's no chance of injections becouse he has a fear of needles, I've started back walking him out hoping this will help but deep down I know this cant go on, I am I right to think I am cruel keeping him in pain :(
 
Is he insured ? Have you spoken to your vet about options ? IF he is sedated will he take a needle ?

My TB had a course of Tildren in January for spavin on his hock and due to other injuries he had more time off than needed but is now coming back in to work and is moving really well.
