horse has hypoproteinemia and edema.What's wrong?

Hi, My loan horse is a poorly boy. He is about 20 years old and has been fit and healthy up to now. A few weeks ago he was really poorly, high fever, not eating and really lathargic. He had antibiotics and blood tests showed he was anaemic. So vet gave him vitamin injection. The other day the vet came and noticed he had swelling under his tummy. He was concerned about heart disease and so referred him for an ultrasound. He took more bloods and now it seems he has hypoproteinenemia and has to have a full blood transfusion urgently. What could be wrong with him? I am trying to be optimistic and hope it is all related to the illness he had a few weeks ago?
