Horses tail falling out?

My horse's tail started to fall out September/October time after he had a light bout of colic, after this his tail just started to fall out and we thought it may have been the stress of it all. But 6 or so months later and it still isn't getting any better, we cut a few cm off of his tail to maybe renew the growth of his tail but that didn't work, he is out for most of the day on grass and is in at night, feed consists of calmers

What are you actually feeding him, is it balanced. Also how short is the tail now, and do you brush it?

His feed consists of He is out for most of the day on grass and is in at night. His feed consists of Equivite original, Onecup feed supplement (both vitamins), calmers, a half scoop of alfa, 3/4 of calm and condition 2x a day and adlib hay. The shorter part of his tail is just above his hocks and the other half is just lower than the hocks and I tend to brush his tail with a dandy brush or body brush.
