How can I stop him Head Snatching? Its driving me INSANE!!

I was wondering if any of you can give me any hints as how to stop my horse head snatching when i'm hacking out.

He is normally okay on the way, but on the way back he starts snatching the reins, and to be quite honest, i've had enough as it is rude and is a pain in the butt He also does it a lot when i'm talking to someone, and I know it is because he is impatient, but he needs to learn some manners, it isn't as if he is a young horse, he is 13 this year, and I was just wondering if anyone could lend me some tips so save my sanity?

Have you had his teeth and back checked first of all?

We had his back checked not long ago and his teeth about a year, but he is fine when i'm going away from home, its just coming back towards that he starts doing it

Well his teeth are probably due being done again anyway, but it sounds like he wants to get home for his dinner. So my plan would be, when he snatches, turn around and go away from home, so it doesn't get him to where he wants to go.
