How do I do this? Insurance claim form...

My insurance form has to be completed by the vet (well part of it does). I have had treatment from my normal vet to the tune of 130 (I have 120 excess on policy, but this still needs to go on the claim form), and surgery+lameness investigation through a clinic, to the tune of about 2500ish. We have to deduct a bit from the 2500 that can't go through the claim, as the insurer will not pay for livery and nursing care.

Anyway, how do I deal with getting the form filledin when I have two different vet hospitals dealing with this. Do I send it to my normal vet to fill the whole thing in, or do I send it to the clinic then pick it up and take it to normal fill to do their bit and then send it off? I don't understand.

I ideally want to get this in by October when my new year starts. I hear Scot Equestrian have been taken over by Petplan and my friend had some bother with Petplan - anyone else had trouble with them?

Petplan seemed to go through a few err 'issues' a few months back where they seemed to be stalling over payment or refusing for no particular reason... personally I suspect they had a cashflow problem that they were trying to cover up! However, I haven't heard of anyone having problems recently...?
I think you get one vet to fill in the claim form, but you add both bills together and put it on the invoice. You may be charged two excesses if they refuse to include them in one claim... though if it is presumably treatment for the same condition, then it hsouldn't be a problem.
 
Hi, I had same sort of thing last year when my mare went in for some work with my normal vet then was referred to another. I sent my form to my vet who filled in her bit then sent it back. I believe the referred vet sent her all the reports so she sent them all on together.

Does that make sense??

i think i had hospital fill mine in - didnt bother removing things from bill as claim assessor will go through all that and decide what can claim for. form should ask for own local vet details too i think. havent had any problems but been a year since claimed and was paid no problem - 5k i think
 
When my horse was first ill i went to one vet who said sound. i then took second opinion who said lame. Referred to Rossdales = £5000 bill for operation.
So i wrote to KBIS (who were brilliant

) and explained about swopping vets. My vet filled in all the details. Rossdales sent a detailed reoprt that was attached. 5 days later cash in hand (Well cheque in post)
Petplan, wouldn't touch anymore sorry - they refused to pay out on death of my mare & wouldn't even pay for disposal although it was covered. Then had the nerve to send me a reminder on renwal 3 montsh later.
 
Yes sorry - forgot to say dont worry about taking things off the bill, your insurance company will do this.

Also, I am with Amtrust and all livery and nursing fees paid (saved me a small fortune last year!!) Just to keep in mind if you do change at a later date...
 
Really worried they won't pay out because this is a previous existing condition (although no one knew about it and it was only found because of a meniscal tear causing pain in the area, so they xrayed and found all the other damage and had to operate to remove damaged bone and tissue).
 
I wouldn't worry - like you say - no one knew about it so they cant say they wont pay when you didnt know about it.

I know how stressful it is tho. I hated it.

I am fairly new on here so not sure whats been wrong with your boy/girlie but i hope he/she gets better soon.

Send it to your vet - they should have everything from the hospital.

I've often thought "pre-existing" is a strange word as most conditions will have been there a long time before they are known about. "Previously diagnosed" would be a better way of describing it. I wouldn't have thought you would have a problem especially as he passed a vetting a year ago.

A vetting wouldn't show this kind of thing anyway, because the meniscal tear was caused by an injury and this tear caused lameness; only by investigating that did they find the OCD. He has never been lame before this and short of xrays of the stifle, no one would have known about it.

I think yes I will send it to my vet. There is a form to complete on the back (by the vet) and then they will attach the referral report and any invoices.
 
Send the main form to your own vet. You may need to get a copy of the report from the referral vets to send to your insurance company. I am going through this at the moment and both vets thought the other had sent the report to me/my insurance company! I got the referring vets to fax the report to me and faxed it straight to the insurance company. It is also worth keeping copies of everything too.

It is also worth ringing your insurance company to check they have what they need and how things are progressing.
