How to stop a dog licking his inflamed...ahem...privates...?

My neutered (neutered ages ago) Rottie somehow got an inflamed errm penis... Went to the vets, put on ABs & anti inflammatories, it got pretty much back to normal then flared up again. They did an ultrasound under sedation in case there was a splinter or grass seed etc...nothing. Urine sample fine. More ABs & anti inflammatories but they concluded that it's flared up again because he's licking it, so this time he has got to wear the Cone of Shame until 5 days after finishing the pills. It hasn't really been improving that much, & this evening I caught him busily licking himself despite wearing a properly fitting cone! What do I do now...?!!

Should I muzzle him so he can't lick, or would he just bash himself instead (as it were...)...? Do the surgical pants work? He can't stay on pills forever but I don't know what we can do to break the lick cycle!
 
I have to get extra large cones for the sighthound, customise the neck end to narrow it and thread a collar through the neck end to make sure it cannot be dislodged. If you use a muzzle you may need to block any holes at the front with something like Gorilla tape.
 
I have cut up a old pair of pj bottoms in an emergency
Won’t work with biters obviously 😅

You can get knickers like disposable type things for male and female dogs.

I got some period pants for Mave when she was in season I only used them when she was in our lounge in the evening, they didn't bother her at all you can get washable ones or pants with throw away liners.
 
Thank you...lots of great ideas. We are off to the vets yet again on Tuesday, so if he still needs the cone we will try inflatable donut + period pants! I was looking at the donuts & thinking that they would be completely useless, but if used as well as the cone rather than to replace it, I think it would work really well. Oh, and now he's limping too. Yay, I wouldn't want the vet bills to stop pouring in...
 
At the vets, we were able to get an inflatable donut so he is now delighted to be sporting cone + donut… It’s doing the trick though! Back to the vets on Monday to see if we can finally get off the pills merry go round…
