I need help filling out my day ticket entry form!

Hi i need help filling in my day ticket entry form. Iv stuck on my stickers, and written on what i understand. The bits i dont understand are

order of preferance this is saying if i have more than one horse and then says list below your prefered order of riding?
do i have to fill this out as i only have 1 horse?

the 2nd part is the other side, where it says qualifications for finals of sponsored series, then it has date, event, class/level, dressage, sj, xc, placing for both horse and rider. What do i write in this?

and then the box underneath that saying communications to (if not rider overleaf) then its got name address postcode.
do i need to write in this as i am the rider?

This is my 1st time doing this so im quite confused, and it says if you dont fill out the boxes then it will be sent back to you, and i want to get it done quickly so i dont get balloted out!

Order of preference - don't put anything in there if you've only got one hors

The other side of it, with the qualification bit, I've never put anything in there either!

Then the communications box you would use if say, you had someone else riding your horse, but you as owner, wanted all the paperwork to go to you. or if your entries are done by a secretary, or something similar! So in your case, as your the rider doing your own entries, then leave that box empty too!
