I have this. After several unsuccessful attempts at treating with beta blockers I had cardiac catheter ablation to determine the cause and laser the area of the heart to fix it. After 2hrs of surgery without anaesthetic (which was not as bad as it sounds) they were unable to find a cause and have now resorted to using an angina drug to treat it.
My restin hr is 40bpm and would rise to 180 just on walking and 220 if I rode. I found a heart monitor a useful way of stopping before I did too much but often very little was too much. I take 10mg of ivabradine now which seems to keep it reasonably under control.
They determined that the cause of my IST was from riding and exercising when I had chest infection/pneumonia and it put too much strain on my heart.
I hope they can treat with medication but if a catheter ablation is needed don't worry, it's uncomfortable rather than painful and really interesting to watch.

My HR has never gone quite as high, max it got to was 148 but this was whilst lying in CCU but I had chest pain/dizziness. Mine was also triggered by a virus it seems!
Its hard to say if I have chest pain from it as my lungs are damaged and I take morphine continuously but the only symptoms I really struggled with was the exhaustion but the drugs helped a bit. Ivabradine is primarily used for chest pain so that might be an option for you. Are you able to ride?
