Insurance claim help please

I'm making a claim for some vets fee's following an accident, I've got the claim form but am stuck on a few questions:-

a - has your horse ever seen a vet?
Well obviously yes, she's been insured with this co for 18 years, and I've never claimed before, do I say yes, and then qualify it with "routine visits in the last 6 months" ?

b - where you responsible for the horse at the time of incident?

Well I'm her owner and responsible for her, but the YO was handling her when the accident happened - what do I reply here?

I had this too - I put "yes for routine vaccinations only" And the other question - yes you were responsible for her! I think that question is more aimed at those horses who are on loan.
 
Unless the question asks whether they have seen the vet OTHER than for routine vaccinations (in which case put No) the just answer "For routine vaccinations only".

As for the 2nd question, yes you were responsible for the horse at the time as, as the other poster mentioned this is more to do with loan situations. If you want you can mention the horse is on a livery yard and I don't see it necessary.
