Pictures Itchy inner thighs

Hi, firstly I have tried and tried to attach pictures but whatever I do it says file too big!! I am happy to send someone a email of the pictures to have a look if you want to pm me……

‘P’ has always been a bit of an itchy boy but only on his inner thighs and around his sheath. He has been particularly bad just recently and will back up against anything to try and rub between his back legs. I have now noticed some bald patches appearing but no cuts or scabs. He is a feathered cob but his legs/feather are clear, he doesn’t seem to be itchy on his legs at all. So stupid question but could it still be mites if he’s not itchy with his legs or anywhere else? His coat and skin looks fine everywhere apart from around sheath and inner thighs. I clean his sheath roughly once every other month as he does get quite a gunk build up, just using warm water and water based lubricant. I had the bean lady out last year just to make sure I was doing a proper job and to check for anything dodgy around that area as he is a grey cob and had a small lump under his dock.

He is the boss of the field and will go up to his companion and get him to groom around his itchy area.
He has had dectomax injections in the past and I have used deosect before but to be honest I’ve never really noticed any difference in the area he is itchy. Any thoughts or theories would be much appreciated xx
 
How old is he? my old girl became really itchy when she developed cushings! I also heard that alfalfa can cause horses to itch! but if he's a fluffy boy my first port of call would be to clip him and see if that helps.
 
He is 18. Hadn't even thought about Cushing's to be honest as my old boy 'B' has Cushing's and P isn't showing any of the symptoms he did, but I acknowledge every horse can show differently. Definitely something to think about thank you.

Yes he is a unclipped wooly mammoth at the moment so I'll try clipping him (when he dries out) and see if it helps.

He has no alfalfa in his diet as I've always avoided that with him. He is fed fast fibre, turmeric xtra and a all-round vitamin and mineral supplement with 'dunked' ab lib hay. Bedded on straw pellets with shavings on top over night.
