Hello for one of my projects I need to get a survey done about laminitis please can you complete for me

1. How often do you come across it? 0-1 times a year 2-3 times a year more .. Times a year
2. Which month it occur or usually occur?
3. If yes what type/breed of horse(s) General horse information? ....................................................
4. Is your horse prone to it? Yes/no
5. What do/would you prefer to use to treat it?
6. How are you trying to prevent it? .
7. How severe is it usually: mild moderate severe
8. What type of soil was the horse(s) grazing on? Chalk Sand Pete Clay other .
9. At time what was grass quality like? Poor good very good lush
10. Reason for laminitis? Pasture related Mechanical stress related Cushings other
11. Where was horse kept? Stable pasture combination
12. At time where was horse kept (i.e. Winchester)?
