Loan Agreements Help etc!

So, our beloved 19 year old mare has retired from polo this season and is going on loan on wednesday... I think I have found the perfect home for her (rang references!), kept at home with lots of land, lovely knowledgeable people who are part of a PC etc. etc. but obviously we need to make some sort of contract type thing...
What do you put in it? We've never put a horse on loan and so we are new to this and as we've had her nearly longer than me she means a lot to us so we want her to be safe etc etc.
Any help much appreciated!
