Loan agreements

After 5 months of trying to sell Alfie, 2 different sales livery places, my current yard where i keep other 2 has said I can fetch him home till horse sales pick up, and I am going to let him go on loan to a young event rider as I have no rider now.
I would like to pick horse and hound forum folks brains a bit!! where do I source a loan agreement as I have no idea of what to put in one it I made it myself, Any advice on pitfalls of loans? All comments welcome.

Yes the BHS one is a good one to use. I have used it for two of mine as one of mine is on permanent loan to me, I got the agreement as wanted something in writing for both parties as my horse was injured when I had him and had that fear of was I being used to care until 100% again. Obviously that was not the case but it does happen in the horse world. I also used it when I had to put my TB out on temp loan last winter so as to have it clear in writing what I expected of the person who would be looking after her.

When I loaned an ex-racer I not only had an agreement but I also included a questionnaire with questiions about any bad behaviour previously, any health issues etc. I got the owner to sign and date it. The owner was quite happy with the questionnaire she said that it stopped any misunderstandings. It wasn't a guarantee but if she had refused to fill it in it would have made me reconsider loaning him.
