medical cards/forms

I have just bought my medical card, only thing is it doesnt come with the insert with all the infomation about health questions and that? anyone know where i can get that from or do i just wait and fill it out on the day?

i normally just get it from the xc course and havent had my own lol

You could either contact BE and ask them to send you an insert

Or just write out a few details on a piece of paper; your name, name and contact number for 2 other people, if you have any health issues such as asthma, hay fever, diabetes, epilepsy etc, religion (if applicable to medical treatment) , any regular medication you may take, if you are allergic to anything and date of last tetanus injection
(That's what's in my PC one anyway)
