Nomination form, please help!

Just had the nomination form through from the stud but we are a bit confused with the first bit. It says
I_________agree to take a nomination to___________at £____ inc of VAT, and exlcusive of transporation costs.

I'm guessing the first gap is where my dad needs to put his name, and the last gap is the amount but what do we put in the middle bit??

Could be, we thought it was either that or the name of the stud but guess it is more likely to be the stallions name! Thanks
