A year ago my 20 year old IDxTB was on borrowed time following laminitis and a very bad abscess. He was so poorly that I came very close to making that decision on more times than I care to remember. However we have spent a long year on restricted grazing (a b*gger to deal with when you have a winter poor doer) and regular trims (initially not regular enough) from my trimmer. Sadly I don't have pics of his shod feet, just some taken about 6 weeks after they came off, then a set of pics from 3 months ago and a few weeks ago (still growing out initial flare that we don't mess with!)

Obv still long way to go but heels seem to be improving to me finally!?
 
Yep, the heels look a bit better, and crucially it looks like you're getting control over the rippling on the hoof wall.

Given his age and that he's a poor doer, have you thought about testing for metabolic conditions? Cushings would be top of the list. It would also explain why you've had such a nightmare trying to keep the sugar intake down.
 
Ah fair enough. Perhaps he's just very sugar sensitive. I've found in the past that after an event such as laminitis, or anything which messes with the hind gut in a big way, the horse can struggle with sugars far more than it did before hand, so maybe it's just that.

The main thing is that you're heading in the right direction :).
