Passport - Changing owner/ horse name

First time I've done this process - Once I've taken over the passport what form do I need to fill out to transfer ownership?

Also, I'm thinking of changing her name, can this be done at the same time? ... What if her current name is on the passport?

Different passports vary, so find out what passport agency/ breed society the horse is registered with and go onto their website to find out what to do. Changing the horse's name is usually more expensive.

My mare has an AES passport. I just looked up their website to see what I had to do. They didnt ask for proof of purchase. I think it cost £28 so not too expensive and I received it back within 7 days.
