Pastern Dermatitis Treatment

I have a 16.3hh ardennes draught horse, which has had pastern dermatitis we believe for the last 3 years (due to neglect in previous home). We found it last summer, and have had the vet out to see the condition they gave us a two week course of anti-biotics for him which took effect instantly but as soon as the course finished we were back to square one. At first he had maggots in his legs and flys where dropping in and out. We have been treating it since we discovered it, and if it flares up bad we call the vet, everytime now when they come out they dont even get up close & look they look from a distance and say were doing great, which really isnt no help! Hes got it on all 4 legs, his back right & front left are the worst affected legs. There are days he comes in & will not put his foot on the ground, he doesnt stable at all even for short periods of time. So, i was wondering has anyone ever had this problem before with their equine friend? How did you treat it? How did you keep flys off your horses legs? We've tried deosect, naff off fly repellent, we've tried putting dishcloths over the affected legs, but he gets them off! I would upload a picture, but it is a very bloody/mucky picture of when we first discovered his problem. Any advice anyone has would be greatly appreciated! We're at wits end with out fellas legs, treating them with the treatment having little/ no effect. Thanks
 
neem oil cream is good for keeping fies away,have a look at Protek Biochem on facebook,they may be able to help,had great results with their shampoo.
 
Will look into that thanks, just looking to see if anywhere does fly/sun boots in his size, but cant find any anywhere :( only in america and thats miles to far
