Pharmacy made mistake in horse drug

Just been released on yahoo news that the 21 polo ponies that died at the weekend in florida was due to the pharmacy making a mistake with drugs, took the poor horses upto 3hrs to die, sorry dont know how to put a link up to the story, maybe some one can.
 
Lots of trouble coming there way - the drug combination is illegal in the US, and pharmacists are not allowed to mix drugs, merely mask tastes and change the form it is being sold.
 
A lot of trouble for the pharmacy and the vet - there might be some drug testing for the polo ponies, but I am not sure how it will work.
 
Horses are given incorrect medication all the time - at a vet school in the US a student (who was 3 weeks from graduation) put feed intended for an NG tube into an IV catheter. How someone does that I do not know.
