Possible Neurological Disorder?

I have owned my 18 year old welsh gelding for almost 6 years now. He has always been hot, tossing his head at fences, and shaking it back and forth when asked to collect. Over the past 6-8 months, he has began to toss his head in the most bizarre way. It started only at feeding time, then progressed to when he was being led, longed, and in the pasture. He NEVER does it under saddle, and although hot, he has very good manners! I’ve done tons of research and the only thing slightly related to what he does are various neurological disorders. Have you had a physio check him over for anything that may be tight or causing him discomfort? If you have and you're worried I'd ask the vets opinion again or go to another vet for a 2nd opinion

It sounds it could be head shaking which can be caused by a number of things it's very hard to pin point what causes it, one if mine is classed as a mild head shaker he rubs his head on his legs or on other objects, vet thinks his is caused by nerve damage in his face caused by reoccurring guttural pouch infections.

He gets regular physio and massage sessions. They said his poll was tight, but was likely caused by the headshaking, not the reason he shakes his head.

Did they explain why shaking his head up and down would make his poll tight?
