Proximal and medial are terms that describe where on the horse they are talking about. Proximal is towards the body of the horse, medial is on the inside of the body. An exostosis is a bony enlargement like a splint.

This says to me it is a splint at the top of the cannon or shannon bones on the inside of the leg.
I dunno if this is what your refering to but thats what i thinbk when someone says that to me!

I would say that this is likely to be a splint/bony growth at the top of the splint bone, but in towards the middle of the horse (so the left of the horse's near fore if looking at it head on)

Cannon bones front legs, shank/shannon bones hind legs.
An exostosis can be a splint, it can also be as a result from a direct blow to the bone causing bruising.
