Haha, nope. Once went to vault on when horse was cantering along, in the middle of games. Hit my pony, bounced back off and landed on the floor again. Whoops.

Needless to say, I lost that one.

I once did it at a gymkhana at college when mounting a shetland bareback. My team then lost as we couldnt stop laughing. Another team member also fell off as he was laughing too much.

Nope, never have! I did once forget to do the girth up thoughm which resulted in the saddle sliding round underneath when I was getting on.

Never done that but did put the wrong foot in the stirrup mounting once when I was very young - did it completely without thinking and was rather surprised to discover my horse's head had been replaced with a tail once I was aboard!

Not quite the same but (only this evening) I valted onto Max (13.2)...except I vaulted high enough for Sparky (17) and landed on my arse on the far side with Max looking at me in a bemused manner!

I gave someone a leg-up once and threw her clean over the horse ...

I tightened the girth, and used a mounting block, the horse started to walk away when I put my foot in the stirrup, the saddle started to slip ... Ow!!!
