I hope I can be of help. My daughter just bought one for our "fat-head" RID and we got the second largest one (not sure of what there sizing is). I believe that they run on the large size, so if your horse is towards the top of one size (say full) then get the large Micklem, not extra-large.

They absolutely do not come up large at all, if anything they are small. best advice is to contact Micklem and get measurements and compare to your current bridle. I usually use cob on my 15.1hh ISH and small horse ONLY just fitted. It is very different form a normal bridle as the throat lash sits in a very different position and depending on your horse's physiology will depend on whether it will do up...
 
I found they come up on the small side especially the throat lash. I bought a cob size and had to return it for a large.
 
I've got the standard horse size for my 14h.3 Welsh cob who normally takes a full size bridle. Does anyone who has the micklem not use it for bitless? I want to use mine bitless but have 'mislaid' the straps. If anyone has the long strap for the strongest bitless option in black I would be interested in buying one.
 
I have the standard horse size for my 15.3 tb. who normally has a cob size bridle. They seem to be made for the long, slim head rather than the short, broad head.
 
I tried a std horse for my 16.1 tb , it was too tight, id agree with going up a size imo theyre a bit small, i followed the direction off the net from horsewear on fitting it as well so it was fitted properly.
