Very sad.
Just been sent this from BE:

It is with deepest regret that we announce that Jo-Anne Williams suffered a fatal accident whilst competing at Sapey One Day Event in Worcestershire on Tuesday 17th April.

Jo-Anne Williams (34) was from Llanfaethlu, Holyhead, Anglesey and married.

The accident was at fence eight on the novice course. The horse, Shannandoah, a 14 year old dark bay gelding owned by Jo-Anne with 28 British Eventing points, number 228 in Open Novice Section E, was uninjured.

That is awful, por family. As said it is not expected at a low level, but unfortunately can happen at any level. I assume the horse fell on top of her?
