Sacroiliac, Heads a spinning!

Mare was diagnoses with PSD and Sacroiliac Pain not long after owning her!
Op for N & F in August, Sacroiliac injected, box rest, pen rest = 9 weeks, back in herd mid Oct, turnout approx. 8hrs per day.
Have had regular physio since Oct, pain was at her withers, all the way along spine & on both quarters.
Had a set back November, lame in walk, back spasms, treated with danilon, vet visited at the end of course and was happy for me to bring slowly into work.
Hacked over Christmas, short walks on roads, showed signs of discomfort again early Jan, Lame in trot on a circle (lunging for testing purposes only), Vet sent me some bute.
However I haven't given her any, as with the delay in contacting vet (Vets days off) and Bute arriving in the post, the rest seemed to put her right again, Physio was due so wanted the physio to assess without pain relief, it seems after 3 treatments (every 6 weeks) the only sore spot that remains is over her sacroiliac on her right hand side & her poll, she is still lame in trot on her RH, confirmed by physio.
What wwyd now? I feel out of my depth to be honest.
I know its still early days..its the lame in Trot which is worrying me it goes against my grain to work a lame horse :-(

She has Riaflex supplement Gluc & MSM which I started Mid Dec and I feel is making a difference.

I have been doing poles and straight line walking in hand in the arena, she will walk with her head low and is over tracking equally in walk.

I do stretches, carrot & ones shown to me by Physio daily, use a Back on Track sheet at night and a back massager 3 x per week.

Vet has said more or less get on with it, use bute as & when needed or if I want I can take her in & she will reassess, feel like a neurotic owner to be honest!

So do I continue with poles, in hand walking and start reintroducing short ridden hacks? Do I need to give her Bute while I do this with her being lame in trot ?

Would you consider a Arc Equine ? altho £550 pays for a lot of Physio too! Tumeric???

Its endless, how do you know what to do for the best ??
 
2nd the Right hind lameness to Hind Gut Acidosis issues - search HGA on here as theres a lot of info starting to gather on this particular subject.
I love my Arc machine on myself - it certainly helps pain issues.
