Sacroilic - prognosis

My horse has been nerve blocked this morning and there is an indiction of pain in the sacroiliac region. He is going back for some medication in this area next week steroid injections, shock wave therapy etc then a strict build up fitness programme starting from walking in hand to full work after 12 weeks. I am pleased that some area of discomfort has been identified but really disappointed that it may be a sacroiliac problem as I understand that this can be very difficult to improve and the longterm outcome is not very good.

Has anyone had any experience of this and what is the prognosis?
 
My horse has a similar issue but also has problems in his hocks aswell. Mine is on box rest with 10 mins inhamd walking for 2 weeks then hopefully he will go and work on an equine treadmill to build up his muscles, then it is back to building up his fitness by walk and a small amount of trot. My vet was fairly optimistic about his recovery.
