Septic Pedal ostietis

My friends 24 year old gelding was diagnosed five weeks ago with septic pedal ostietis by my vet after being misdiagnosed by both another vet (bruised sole) and tendon trouble(farrier). Three weeks in to finding him suddenly lame he was started on a large dose daily of penicillan for 5 days then 5 days of both penicillan and gentomycin then five days of more penicillan. He also had Danilon in large doses to begin with reducing to a now maintenance dose of one daily.
He was leaking joint fluid when the second vet visited. The alternative to this treatment was to take him 70 miles to Vet College for debridement under anaesthetic which he was given a 50/50 chance of recovery.
Three weeks in he has improved significantly, but is still slightly lame. He is a much loved friend now, not ridden. Has anyone any experience of this condition? If so what was the outcome?
The source of the infection was thought to be mite related (he is a 16.2 heavy, hairy legged boy) who has been seen to chew at his legs. His owner has tried all treatments imaginable to get rid of the mites.
