It is always best to ask

Yes if at all possible you need to contact insurance company BEFORE surgery. However if it is an emergency and not in office hours or so much of an emergency then no. Too be honest any vet would most likely be able to tell you

If you take your insurance documents with you, then no you don't have to pay up front.

You need to make sure you go with a insurance company that has a good reputation for paying out. Plenty of recommendations on here if you do a search. As above if it is an emergency then you don't have to get permission. But you must inform them as soon as possible. Most vets don't ask for payments upfront, but you could always ask.
