You know that horrible wobbly crest that happens when the pony gets way too fat....

One of mine has a disgusting crest, doesn't help that she's a mare, such not a good look for her. One of my others is IR/EMS so she is treated the same as it's easier. She's always had a crest, somewhat scarred from previous untreated sweet itch so makes it look even worse.

Diet - out 24/7 on very sparse ancient horse grazing, 2kg of old meadow hay in morning/evening, dinner is Lo-Cal/MagOx/Brewers Yeast/Linseed/Boswellia (bit of arthritis to contend with)

Other than the crest she is disgustingly healthy, full of beans, bright eyes, shiny coat, fit as a fiddle despite just weekend work at the moment, barefoot and always has been, lovely tight feet, never pulsey etc etc.

But the crest doesn't budge no matter how slim/fit she is?

Is it really a case of once a crest/always a crest or has anyone found a miracle cure other than Liposuction?

You seem to be ticking all the boxes. If I were you I'd double check quantity of mag ox - its very individual and some do better on rather more than others. Also I appreciate restrictions imposed by winter. Unfortunately recommendation (ECIR) is min 5x30min heart rate raising exercise per week. I ended up treating mine like a dog and taking her everywhere with me. Except work..... But she came to tack room, car park etc, always at sharp walk or slow jog. Don't have that problem now but I do sympathise.

Demoralising isn't it? I've increased the Mag before but it then becomes difficult to get her to eat, despite being a hairy bog native she is rather fussy about food, doesn't like anything sweet other than veg so it's difficult to hide stuff bless her.

Perhaps she'd like one of those fancy neck sweat wraps that they still make and use in the States for show horses - LOL!

One of the ponies at our yard had an enormous crest for many, many years. Due to a laminitis attack he has been off grazing completely for several months, only getting rationed hay (& his minerals in a tiny bit if speedibeet) and he looks like a different pony, all fat pads gone and a tiny little neck. Looks 15 years younger!
