I have a little shettieX gelding who has some problems with his mouth.
Firstly he gets hamster cheeks (food stuch up between his teeth and cheek causing a swelling) And secondly he drops food when he is eating. Ive noticed this with treats and carrots but not grass.
He is quite fat and is 20. He might have cushings.

What should I do?

Def get an equine dentist to look at him. Shetlands and miniatures are well known for having teeth problems and if he's having problems with his food then there is something wrong. He might just need his teeth rasping or there may be something more serious. Some miniatures/ shetlands have off "bites" where there teeth dont align so causing problems when chewing. Only a vet or dentist will be able to advise but id do it sooner rather than later or you will end up with more serious problems if its not sorted.

Yes definitely get an equine dentist or a vet to come and do his teeth. Cos from the problems you have described sounds like he can't chew properly. If you think he has cushings then you will need a vet to diagnose him with cushings and then he can get the right medication to help him with that condition. Mine has cushings and he is about 30 and just had his teeth done, needed a bit of work but now he is fine.

get is teeth checked it sounds like it might be a bad ooth or maybe just a few sharp edges or some hooks all they will do is rasped them down!
